Adipositas und Diabetes mellitus

Die Adipositas ist – wenn eine entsprechende genetische Disposition vorliegt – der wichtigste Risikofaktor für die Entwicklung eines Diabetes mellitus Typ 2. Beide Erkrankungen werden daher auch häufig unter dem Namen „Diabesity“ zusammengefasst. Ein erhöhter Body-Mass-Index (BMI, s. Tab. 8.1) geht im Durchschnitt mit einer erhöhten Mortalität einher. Dies gilt allerdings auch für das Untergewicht. Ein hoher BMI ist einer der fünf häufigsten Risikofaktoren in Bezug auf die darauf zurückzuführenden Todesfälle (Attributable Death) und die durch eine hierauf beruhende Behinderung beeinträchtigte Lebensdauer (Disability Ajdusted Life Years). Er ist damit einer der wichtigsten Gesundheitsparameter. Nicht nur in Europa, sondern weltweit ist der größte Teil der Diabetes-mellitus-Typ-2-Fälle der Adipositas zuzuordnen. Die Milan Declaration 2015 der Europäischen Adipositas Gesellschaft EASO bezeichnete die Adipositas daher als ‘progressive Erkrankung’ und als ‘zentrales Tor’ zu vielen weiteren Erkrankungen (insbesondere den meisten nicht übertragbaren Erkrankungen, NCD). Damit wurde die zentrale Rolle der Adipositas bei der Entstehung von Diabetes mellitus, Hyperlipidämie und Hypertonie einschließlich der Konsequenz einer erhöhten kardiovaskulären Morbidität und Mortalität allgemein anerkannt. Die Weltgesundheitsorganisation (WHO) erklärte die Adipositas zum größten globalen chronischen Public-Health-Problem, das inzwischen die Bedeutung der Mangelernährung deutlich übertrifft. Neueren Berechnungen zufolge könnten im Jahr 2025 bis zu 45 % der europäischen Bevölkerung von Adipositas betroffen sein.
